STEP 1
Contact us directly via phone or our secure online referral form. We’re happy to discuss the suitability of our supported living service.
STEP 2
Our management team will review the referral details and assess whether our service can meet the person’s individual needs.
STEP 3
If suitable, we arrange a comprehensive needs assessment — ideally face-to-face — involving the individual, their representative, and any key professionals. We assess personal care requirements, risk factors, preferences, goals, and compatibility with others in the service.
STEP 4
If our service is a good match, we begin a personalised transition plan. We work at the individual’s pace, with opportunities to visit the home, meet the team, and settle in gradually.
STEP 5
Once everyone is ready, we finalise tenancy arrangements, support plans, and medication protocols. We aim for a smooth, empowering move-in experience with continued emotional and practical support.
